choice between upper or lower jaw surgery which should i do ?

    and yea i agree thinking logically it would seem that the upper jaw would be way more painful , but i guess most surgeons prefer top jaw as its an easier recovery

    Actually I'm in a similar situation myself. One surgeon said I just have to do move my lower surgery and my upper jaw was fine...the second surgeon said it would be more proportional to move my upper as well...although to my untrained eye, I feel like my upper jaw profile is fine as is...I'm sortof torn and at the same time concerned why two doctors would have such different ideas.

    From your picture it seems your underbite was quite similar to mine where you midface looks slighty sunken in which would suggest a upper jaw deficiency. Before my surgery I constantly kept reminding myself of why I was having it done which were to be able to eat properly, speak better (I had a slight lisp) and to be confident enough to smile also the cosmetic benefits as a result of the surgery :)

    Upper jaw advanced by appox 7mm, impacted and rotated slighty on the 19th December 2011.

    You're the guy on right correct?

    I believe surgeons don't generally move the lower jaw back because it reduces your airway but I may be wrong about this. When people have sleep apnea due to a small airway, they advance both jaws forward to open it up. A reduction genioplasty is often done in underbite cases to reduce a prominent chin after lefort and or/bsso but I haven't see many examples of the lower jaw being moved back.

    So doing the upper jaw only in your case seems appropriate.

    Hey ron,
    I'm still presurgery but thought what I might say could possibly help. The shape of your face and the positioning of your jaw is visually exactly like mine. Twenty odd years ago the Lefort I surgery didn't exist and the only surgery available for an underdeveloped upper jaw was the lower jaw push back surgery. I was told this was the surgery I had to have combined with cheek implants (which would offer a better proportionate outcome). Well I didn't want to radically change my face and didn't like the idea of having anything foreign in my face so I opted out. Now here I am that many years later preparing for the Lefort I surgery; I've had two surgeons tell me the Lefort I would be ideal for me. I did do my research and like so many folks on this site... made sure my surgeon mainly performs this procedure and has a good educational background and worked at some of the best hospitals (Harvard educated and Mass General/Brigham). All that said, I'm glad I waited because the operation offered now is far superior to the one I would have had and I am less concerned with how much it will change my appearance. As you get older the discepancy between the upper and lower jaw...looks sharper, the misalignment between the jaws becomes more of a medical issue and you see how much more proportionate your face will appear with the upper jaw surgery. If you are still unsure of your choice, have two computer generated simulations done, they are not perfect but it might help you decide.

    suemc111: so i am assuming that you are only moving upper jaw ?

    does any one have an idea how much change i should expect from 5mm in the upper jaw ? like will some be not be able to recognize me ?

    if you like the way you look now, and have no pain/jaw problems, then I would not get the surgery at all. The surgery will affect your appearance atleast slightly no matter what.

    ron sorry for the delay...yes the Lefort I is only upper jaw surgery. I think Brandon is right, if you like the way you look and are not having any problems don't have the surgery. It took me over 20 years to start the process I am now ready to have it, changes and all. A big part of this is years of jaw misalignment. I do have a question, I can't tell from your pictures, do you have an edge to edge relationship between your upper and lower teeth?
