lexchomp, I suggest that you try negotiating with the surgical center or shopping around for one who is willing to negotiate. Just to give you an example of how much wiggle room there may be: I ended up having my surgery at a hospital instead of surgical center at the request of my insurance company. The hospital billed my insurance company $50,000 and my insurance company only paid $1,800 (that was their negotiated rate). I have no idea why the hospital would charge $50,000 and the surgical center would charge $7,000 for the same procedure. I also have no idea how an insurance company negotiates $50,000 down to $1,800 - that is just absurd and probably doesn't even cover actual expenses. I'm just thankful that I have insurance that covered my surgery.

Your surgeon's fees are very reasonable so at least you have that going for you. :-) Good luck and I hope you are able to find a reasonably priced solution.

On 12/28/11 I had a 4 piece LeFort 1 procedure done to correct an open bite.

What insurance do you guys have that pays for orthognathic surgery?! Im in the US with Blue Cross Blue Shield but I can get a private plan if it'll cover it. I'd prefer to get surgery in the US but it's prohibitively expensive unless my insurance covers it. I thought they all considered it cosmetic?

I'm fortunate to go through life relatively healthy, which makes me inexperienced with regards to healthcare and how the system works. I hope this helps. Rough numbers for LeFort I, US, California:

    Braces: 6600
    Hosptial: 46K
    Anesthesiologist: 2400
    Surgeon: 7600
    Supervising Doctor: 210

Most oral surgeons are dentists with an extra year of training, therefore they need a medical doctor on hand for supervision, unless they are a medical doctor (plastic surgeon).

I found that the procedure was so specialized, that there were no in-network providers. I had to pay the surgeon and doctor fees up front, and file claims with insurance and hope for the best. The oral surgeon's office got permission from my insurance company for the medical necessity of the procedure, and the assurance the procedure would be "covered". My insurance company will pay a percentage based on out of network, but they will pay full coverage if there are no in-network providers within 25 miles, and cost is based on market rate (what other surgeons in the area charge for procedure code 21147). I'm not here to slag the insurance company, just outline some pitfalls and how to address them if you are in the same boat. The phone adjusters look at the hundreds of oral surgeons in my area, and don't understand why I went out of network. The difference is there are oral surgeons who pull teeth, and oral surgeons who saw into skulls. There is nobody in my network who performs the latter. The other pitfall is my insurance company used published Medi-Cal data as a billing reference point, which is a fraction of the surgeon's fee. Medi-Cal is state insurance which has negotiated rates with select providers, not comensurate with market rates.

I'll update as things develop. It's just disappointing to have premium insurance coverage through an excellent corporate employer and still have to iron out the fine points with the insurance company. This is a relatively minor procedure. What would a major illness be like?

This post is a little cryptic. I have to assume the insurance company can access and read this.

Side note: Recovery was easy, still wearing braces, but my jaw works for the first time in my adult life. Worth it.
