Questions.
1. What if my back molars go all the way back along the mandibular bone and there isn't enough space to cut bone.
2. I have a bigger nose with a zig zag...won't that make the maxilla advancement harder?

1. I haven't heard of this happening, but the right surgeon will surely have a backup plan in place and will of course know this from the first examination. Have you already seen a surgeon for a consultation?
2. Again, this is specific to your specific case and surgery, and the surgeon will be able to give you the right answer.

Usually there will be space if there is no space it means there are wisdom tooth taking up the space. For my case I got 1 internal wisdom tooth on each corner of my mouth. So what they do is send me for a surgery to remove them.
They will be able to cut through it and correct the zig zag maxilla don't worry. =) Unless they are noob surgeons =X Which I don't think I will let them do anything to my mouth =X
